Help Me Help You Refer A Client, gives healthcare professionals the opportunity to refer eligible clients to access our essential and life-changing services.

Through community partnerships and as part of the Unite Us network, a national leading care coordination platform connecting healthcare and social service providers, HMHY also provides clients with a continuum of care by:

- referring clients to supporting services

- providing client feedback to the referral partner

- measuring the impact of HMHY services to improve health outcomes for individuals and the community

Free enrollment assistance for CalFresh benefit that provides qualifying low-income individuals with dollars via an EBT debit card to purchase groceries at participating grocery stores and farmers’ markets.

MEDI-CAL

Free enrollment assistance to access Medicare's comprehensive health insurance coverage at no cost or a reduced cost. Medi-Cal benefits include ambulatory patient services, emergency services, hospitalization, maternity and newborn care, mental health and substance use disorder treatment, dental, vision, and long-term care and supports.
